Prescription process for providers

Reach out to Valor to begin
If you are interested in prescribing, put a request in to Valor through our website, or send us a prescription either electronically (preferred) by fax, or over the phone.

Initial screening process
A pharmacist will contact you to schedule an initial phone call and screening. This call will take about 10 minutes and is an opportunity for Valor to learn about your practice. This is an annual screening that will be conducted for all prescribers sending Ketamine prescriptions.

Post-screening check-in
After the initial screening interview, and pending compliance review, if applicable, prescriber may be approved to prescribe to Valor.

Begin to prescribe compounds
Once approved, you can begin to prescribe to Valor. Please note, Valor follows controlled substance regulations and checks all prescriptions through available state PDMP systems prior to dispensing to patient.

Monthly refill requests
Prescribers may choose to assign refills on a ketamine prescription. However, this prescription is not eligible to be put on automatic refill and the patient will need to request a refill each month.

What does "Days supply" mean?
To register as a prescribing provider with Valor Compounding Pharmacy, complete the Prescriber Inquiry form on this page. It will ask you to submit your professional license information and practice details for verification. Once approved, you can begin prescribing custom compounded medications and request tailored prescription forms specific to your specialty. Our team responds promptly to ensure a seamless onboarding process.
Why does days supply matter?
Controlled medications have more restrictions due to laws and regulations. These regulations are to help ensure patient safety. Because of this, prescription orders often cannot be refilled early. At Valor, we can only fill a new prescription AFTER the days supply for the previous prescription ends. For example, if a patient runs out of a prescription after 28 days of a 30 day supply, the pharmacy will not be able to begin to fill the prescription for another two days.
Do I need to request a refill each month?
Controlled substances such as Ketamine are currently not eligible for the Automatic Refill Program. This means you will need to request a refill each month. To do this you can call our pharmacy, have your doctor send us a new valid prescription, or you can fill out a request for a refill online.
How do I request a refill?
When you are ready to refill, navigate to https://www.valorcompounding.com/refill-rx/ and log in through Tabz to submit a request for refilling your prescription.
