Valor operates a non-sterile and sterile compounding pharmacy for patient-specific medications and delivery Services. It is a resource for patients and physicians to learn, prescribe, refill/transfer prescriptions, or request news and information about custom compounded medications. Valor is a California licensed pharmacy and adheres to the California Consumer Privacy Act. It is also multi-state licensed.

In this Notice, “you” and “your” refer to those that visit or use Valor Services.

Collection of your Personal Information
In order to better provide you with products and services offered, Valor Compounding Pharmacy
may collect personally identifiable information, such as your:

    (In order to provide you with the prescription delivery Services, you will need to provide us with certain medical information such as your physician’s name and address, prescription information, insurance information (if applicable), and in some cases, medical history.)

If you purchase Valor Compounding Pharmacy’s products and services, we collect billing and credit card information.
